Ice Bath Benefits: What the Science Actually Says

The best-supported benefits of an ice bath are mood and alertness. Cold water immersion produces a large, repeatable rise in norepinephrine and a slower, longer rise in dopamine, and people consistently report feeling calmer and sharper for several hours afterward. Effects on inflammation and soreness are real but modest and short-lived, and cold immediately after strength training appears to blunt muscle growth. Almost everything else — fat loss, immunity, testosterone, metabolism — is either small, inconsistent, or extrapolated from studies that were never about ice baths.

That is the short version. Below is the longer one, with the reasoning, the temperatures, and where the evidence thins out.

Why we are writing this at all

We run four individual ice baths on a rooftop in Kuta — two at 17°C, two at 7°C — and two plunges in Ubud, at 17°C and 5°C. People get into them every day. Many of them arrive holding a claim they read somewhere: that cold water burns fat, that it triples testosterone, that it cures inflammation.

We would rather they arrive with an accurate picture. The accurate picture is still good. It just has edges.

One note before anything else: the science below describes healthy adults entering cold water voluntarily and sober. Never ice bath after alcohol. Not one drink, not "just a beer." Alcohol impairs the body's ability to sense and respond to cold, and it removes the judgment that gets you out of the water on time. The answer is always no.

Norepinephrine: the strongest finding

This is the one result that shows up in study after study, in different labs, with different protocols, going back decades.

When you enter cold water, plasma norepinephrine rises sharply — commonly two to five times resting levels after a short immersion around 10°C. It rises fast, within the first minute or two, and it does not fall back the moment you step out. It stays elevated for a long stretch afterward, which is why the hour after a plunge feels different from the plunge itself.

Norepinephrine is the molecule of focus and vigilance. It is also, at moderate elevations, associated with reduced anxiety and improved mood. The classic reference points here are the cold-immersion catecholamine work of Šrámek and colleagues in Prague, and the later cold-adaptation literature out of Finland and the Czech Republic. What makes this finding unusually solid is that it does not depend on belief. Norepinephrine goes up whether you enjoy the cold or hate it. The measurement is in the blood, not in a questionnaire.

Dopamine: real, slower, and often overstated in one specific way

Cold water also raises dopamine, by roughly 250% in the most-cited protocol — one hour at 14°C. The rise is slower than norepinephrine's and, notably, it stays elevated for hours after the immersion ends.

Two honest caveats.

First, the headline figure comes from an hour in cold water, not from three minutes. Shorter exposures almost certainly produce a smaller effect. Nobody has run the careful dose-response study that would tell us how much of that 250% you get from a realistic plunge.

Second, "dopamine" is not "pleasure." It is closer to drive and motivation. The reason a cold plunge leaves you wanting to do something rather than wanting to lie down is probably this. It is also why the effect feels sustainable — unlike stimulants, the curve rises gently and comes down gently, without the crash.

Inflammation and soreness: modest, temporary, and with one real cost

Here is where the internet and the literature diverge most.

Cold water immersion does reduce delayed-onset muscle soreness. Meta-analyses in sports science have found this consistently. But the effect is moderate, mostly perceptual, and largely confined to the first 24 to 48 hours. It makes tomorrow feel better. It does not appear to make you meaningfully stronger or faster in the long run.

And there is a documented cost. A body of work — search for cold water immersion and resistance training adaptations, particularly the Australian studies out of Queensland — has found that plunging immediately after strength training blunts hypertrophy and strength gains over a training block. The proposed mechanism is straightforward: the inflammatory signal after resistance training is not damage to be suppressed, it is the message that tells the muscle to grow. Cold water quiets the message.

The practical rule that falls out of this is simple:

  • Cold after endurance work, or between same-day competitive events: helpful.
  • Cold after strength work, when your goal is muscle growth: wait. Four to six hours, or move the plunge to a non-lifting day.
  • Cold on a rest day, for mood and alertness: no conflict at all. This is what most of our guests are actually doing.

The word "anti-inflammatory" also deserves a correction. Cold does not remove chronic inflammation from your body. It temporarily reduces the local inflammatory response in tissue you just cooled. Those are different claims, and only the second one is supported.

What is oversold

We would rather say this plainly than let you find out later.

Fat loss. Cold exposure activates brown adipose tissue, which burns energy to produce heat. This is true and interesting. The quantity is small — the commonly cited figures amount to tens of calories per session, not hundreds. Nobody has demonstrated meaningful weight loss from ice baths in a controlled trial. The research area is real (brown adipose tissue and non-shivering thermogenesis); the marketing conclusion drawn from it is not.

Testosterone. The oft-repeated claim traces back to studies about heat, or about testicular temperature, or to very small samples with mixed results. There is no good evidence that a three-minute plunge raises testosterone in any lasting way.

Immunity. The most-cited work here is the 2016 Dutch trial in which people who finished their showers cold took fewer sick days. That result is worth knowing. But it was cold showers, self-reported outcomes, and the participants knew which group they were in. It is suggestive, not settled.

Metabolic and longevity claims. Mostly extrapolated from animal work or from sauna epidemiology, which is a separate literature with far better data. Do not borrow the sauna's evidence to defend the ice bath.

Depression treatment. Case reports and small open-label studies exist and are genuinely encouraging. They are not randomized controlled trials. Cold water is not a treatment. It is something that makes a lot of people feel better, which is not the same sentence.

Temperature and duration: what to actually do

The controlling variable is total cold dose, not bravery. The often-quoted target from the cold-exposure literature is roughly eleven minutes per week of uncomfortably cold immersion, spread across two to four sessions. That is the whole week. It is much less than people expect.

Here is how that maps onto the water we actually have.

17°C — our entry temperature at both venues. Cool enough to trigger the response, mild enough that you can stay in for three to five minutes and breathe normally. This is where first-timers should start, and where many regulars stay. There is no prize for skipping it.

7°C — the two colder baths on the Kuta rooftop. One to three minutes. The gasp reflex is strong in the first fifteen seconds; the whole skill of cold water is letting that pass without fighting it. Long, slow exhales. Shoulders down. If your breathing has not settled by thirty seconds, get out — that is information, not failure.

5°C — the cold plunge in Ubud. Sixty to ninety seconds is plenty. Past that you are not gaining more benefit, you are gaining more risk. Get out while you still feel entirely in control.

Three further points that matter more than the numbers:

Submerge to the neck if you can. Hands and feet contribute a disproportionate share of the discomfort, and keeping them out changes the dose more than people realize.

Do not force a hot shower immediately afterward. Rewarming on your own — sitting, breathing, letting the body do it — is part of the effect. On our Kuta rooftop that means the open-sky beanbag area. In Ubud it means the fire pit, or the yoga shala.

And if you are pairing cold with heat, the order is heat first, then cold, then rest. The rest is not an intermission. It is where the parasympathetic switch happens, and it is the part people cut short.

Who should not do this

Cold water immersion raises heart rate and blood pressure abruptly. If you have a cardiac condition, uncontrolled hypertension, Raynaud's, or are pregnant, ask a doctor first — not us. If you have any history of fainting in cold water, do not plunge alone.

And again: never after alcohol.

Honesty about the water

Our plunges are chlorinated and managed daily. We say this openly because some places imply their water is purified by some gentler-sounding method. Ours is not. Chlorine is what keeps shared cold water safe in a tropical climate, and daily management is what keeps the chlorine at the right level.

What is left when you subtract the hype

Strip out the fat loss, the testosterone, the immune claims, and you are left with this: a cheap, drug-free, three-minute intervention that reliably improves mood and alertness for hours, modestly reduces next-day soreness, and asks something of you that nothing else in your day does.

That is a good deal. It does not need to be more than it is.

Most people, after a few visits, stop caring about the mechanism anyway. They come because of how the rest of the afternoon goes.
